This position is located in the Health Care Homes (HCH) section of the Health Policy (HP) Division of the Minnesota Department of Health.
The purpose of this position is to lead the development and implementation of the Statewide Learning Collaborative as part of the HCH component of the Health Care Reform legislation passed during the 2008 legislative session. This position is responsible for curriculum planning, program development and implemention for the HCH Learning Collaborative (LC) and Learning Coummunities. The position will work closely with the HCH, State Innovation Model (SIM) grant team and stakeholders to implement and evaluate the training and educational components of the LC for HCH and SIM populations.
Three years of advanced professional experience implementing and coordinating a regional or statewide health promotion/education project with responsibility for budgeting, setting objectives, selecting methodology, designing content and evaluating effectiveness.
A Master's degree in Health Education, Adult Education, Mass Communication, Journalism or closely related field will count as one year of professional experience, a Bachelor's degree will count as six months.
Requires occasionally lifting such articles as file boxes and heavy hand tools or heavier materials with help from others and/or lifting and carrying light objects frequently. Even though the weights being lifted may only be a negligible amount, a job is in this category may require walking or standing to a significant degree or may involve sitting most of the time with a degree of pushing and pulling of arm and/or leg controls.
A Master's degree in public health, nursing, health education or related
Knowledge of, or experience with the operations of HCH, primary care clinics and patient/family centered care.
Experience in program planning, facilitation and curriculum development.
Knowledge of adult learning principles.
Knowledge of quality improvement measurement and change strategy techniques, especially related to educational programming.
Experience facilitating workgroups and multi-disciplinary task forces.
Experience leading development of a comprehensive educational project.
Experience executing and monitoring contracts and grants.
Familiarity with design and use of online survey tools.
