Job Description:   This posting will be used to fill five positions at one of the following Department of Health district offices: St. Paul, St. Cloud, Bemidji, Fergus Falls or Mankato.

The positions will conduct onsite surveys of state licensed home care providers serving housing with services settings and individual's residences. They will provide surveillance, guidance and training to licensed home care providers to assure a level of quality services, quality care and quality of life based upon compliance with State and federal laws and rules related to nursing and health services.

Responsibilities include:
Evaluate home care providers, alone or as a member of a team, to ensure compliance with State and federal laws and rules related to provisions of nursing and health services.

Provide training to home care providers to prevent violations related to applicable home care regulations and standards of practice.

Compile and record information to document conclusions reached and ascertain the degree of compliance with the home care licensure regulations.

Make periodic unannounced follow-up site visits to home care providers to ascertain compliance with correction orders.

Participate in legal proceedings regarding correction orders and issuance of penalty assessments.

Participate in continuing education programs to develop and expand working knowledge in the health care field and maintain current licensure as a Registered Nurse.

The positions require extensive periods of standing and walking during surveys and the ability to lift/carry and load/unload a laptop computer and surveyor documentation/reference materials for use during onsite surveys.
Minimum Qualifications:   Must possess and maintain a current Registered Nurse license in Minnesota.

To be considered, applicants must meet one of the following requirements:

1) Two years of professional experience as a Registered Nurse in a health care setting with primary responsibility for assessing nursing and health needs and experience documenting and writing reports; OR

2) Three years of professional experience as a Registered Nurse in a health care setting.

A Master's degree in Nursing, Public Health, Health Care Administration, Psychology, Social Work, Medical Field or closely related area may be substituted for one year of experience; a Bachelor's degree in one of the above areas may be substituted for six months of experience.

Oral communications skills sufficient to interview staff, residents, clients and patients.

Critical thinking skills sufficient to collect and analyze information, identify areas of focus, and make decisions based on evidence gathered.

Listening skills sufficient to ascertain level of compliance in meeting resident/client needs.

Interpersonal skills to develop and maintain effective working relationships, inform providers of deficiencies and other pertinent findings, and participate in legal proceedings.

Written communications skills sufficient to document concise, accurate and complete information and conclusions.

Computer skills sufficient to document findings and conclusions.

Requires occasionally lifting such articles as file boxes and heavy hand tools or heavier materials with help from others and/or lifting and carrying light objects frequently. Even though the weights being lifted may only be a negligible amount, a job is in this category may require walking or standing to a significant degree or may involve sitting most of the time with a degree of pushing and pulling of arm and/or leg controls.
Requires a Class D Driver's License: a single unit vehicle with a gross vehicle weight of less than 26,000 pounds. This is a basic driver's license. Operators may also tow vehicles/trailers as long as the a gross combination weight does not exceed 26,000 pounds.
Preferred Qualifications:   Experience in a Home Care or Assisted Living environment.

Ability to apply nursing and health knowledge and clinical expertise to the review of providers.

Knowledge of the evaluative processes and relevant federal and State laws and regulations and ability to interpret these laws and regulations.

Knowledge of home health care rules and regulations.
