Job Description:   This position is located in the Health Care Homes (HCH) section of the Health Policy (HP) Division.

The position provides support for implementation of the HCH initiative within the broader context of Minnesota's health care reform. This position provides technical support for the development and maintenance of the HCH database application that supports certification and recertification. This position provides technical training to staff and customers to ensure that program operations run smoothly, assists in development, tracking and monitoring of grant and contract activities and financial processes, and participates in other aspects of HCH and State Innovation Model (SIM) activities.
Minimum Qualifications:   Knowledge of computer software programs and applications and/or web-based database systems sufficient to maintain and administer programs, train others in retrieving data, and work with information systems technical professionals on program or application upgrades or installations.

Knowledge of health program standards and document control procedures sufficient to support professional needs and respond to inquiries and requests.

Working knowledge of Microsoft Word, Outlook and Excel.

Knowledge of administrative operations or fiscal record systems sufficient to interact with others internally and externally in coordinating complete record systems, adhere to documentation requirements and timelines, and maintain and update records.

Knowledge of information and record keeping systems and processes.

Word processing, typing and proofreading skills sufficient to create and edit training materials, technical reports and other documents.

Oral and written communication and human relations skills sufficient to edit and synthesize tehcnical reports and documents and converse with coworkers, other division and department employees and customers/clients.
Requires occasionally lifting such articles as file boxes and heavy hand tools or heavier materials with help from others and/or lifting and carrying light objects frequently. Even though the weights being lifted may only be a negligible amount, a job is in this category may require walking or standing to a significant degree or may involve sitting most of the time with a degree of pushing and pulling of arm and/or leg controls.

Preferred Qualifications:   Knowledge of MDH and HCH and relevant operations of the HCH program sufficient to maintain internal and external relationships.

Experience executing and monitoring contracts and grants.

Organizational skills.

Ability to work in an ambiguous, rapid paced work environment with decision making at many levels.

Ability to broaden depth and scope of knowledge by asking questions, searching for information, reading relevant literature, seeking out and attending training opportunities, and applying new concepts or technologies quickly and efficiently.

Ability to stay current on issues, solutions, technologies, and professional approaches that impact the HCH program.

Ability to establish and maintain effective working relationships with HCH staff, external and internal customers and the State of Minnesota's central IT organization (MN.IT) staff.

Ability to develop ideas and recommendations and present them effectively verbally and in writing.

Ability to develop and monitor detailed work plans and communicate deviations to leadership.
